Make an Accidental Injury or Specified Accidental Injury insurance claim here.
If you have experienced an accidental injury, and have Accidental Injury or Specified Accidental Injury cover you may be eligible to lodge an accidental injury claim. If you are uncertain about your coverage, please contact us or your adviser to discuss the details of your policy.
To proceed with your claim, download and complete the claims form. Once you have your documentation ready, lodge a claim via one of the below methods. We will call you within 5 working days of lodging the claim and aim to update you on the progress of your claim every 7 days.

Any medical information regarding the accident
Specialist reports
Hospital discharge summary
Radiologist x-ray report (describing the finding of the x-ray)

We pay the money as directed by the owner(s) of the policy. It is important to regularly review the ownership of your policy with your financial adviser if your circumstances have changed.
We process and pay any claim refunds as quickly as possible – usually within 3-5 working days.
